Abstract

RCM Java Racing is a exhaust industry whose products have spread to various regions. The use of tools in the production process of making exhausts is still classified as simple equipment in which the operation is still manual, with high production levels of course it can cause accidents caused by humans. In the 2021 period there were 77 incidents of accidents in the production department caused by the human factor. To overcome this, it is necessary to analyze work accidents using the Human Factor Analysis and Classification System (HFACS) method to find out the main causes of work accidents based on human factors. The results of the analysis using the contingency coefficient test obtained a relationship that influences each other at all stages of the HFACS, namely for the unsafe act stage is a decision error; for the prerequisite stages of unsafe acts are the physical environment and technological environment; for unsafe supervision stage is the failure of the correct problem; for organizational influence is on organizational processes. Therefore, it is necessary to improve at all stages from level 4 to level 2, so that it can control the triggers for work accidents at level 1.
