Abstract

Medical records store both recorded and recorded information about the patient's identity, medical history, physical examination, laboratory test results, so that healthcare services to patients require speed in obtaining data. Berkah Medika Clinic faces obstacles in managing outpatient patient data because the medical record system is still manual and not yet computerized. The accumulation of file problems is due to the lack of the process of storing medical record files and the lack of a medical record file retention information system also exacerbates the situation. Based on these problems, this research will design a web-based medical record system using the Rapid Application Development method. The application in this study was created using a framework from the PHP programming language, the Laravel framework, and using the MySQL database. Then the application is tested with blackbox testing, then testing the success of the application using a questionnaire for respondents and testing the validity and reliability of the questionnaire with SPSS. The results of the blackbox testing state that the system can function properly, while the results of the system testing received a score of 86.3%, the results of the validity test of the questionnaire stated valid 100% and the results of the reliability test of the questionnaire showed a Cronbach's Alpha value of 0.860 and can be stated that the questionnaire questions are consistent or reliable.
