Abstract
ABSTRACK Our satisfaction survey customer applications is an android-based applications that are in use to assess performance against an employee who is tasked with entering employee data on duty as well as suggestions and criticisms to support reason the survey on a division or function at PT Pertamina persero MOR IV. In practice this field work designing application system design method used Waterfall because in the design of the system on the model of the waterfall has sequential stages giving him the most simple system design method in comparison with other methods. The software used to create this customer satisfaction survey Application is Android apps Studio and PhpMySql. Keyword : Aplication, waterfall, survey, Android Studio, PHP MySql.
