Abstract
The KAOSEDHEWE Convection is a UMKM engaged in the textile sector, by producing clothing, such as t-shirts, shirts and jackets. Every production process must experience problems with the quality of the products produced, defects in screen printing t-shirt products are the most compared to other products, so that tshirt products are the main goal in implementing quality control to identify and analyze the process of screen printing t-shirt production and find out the factors causes of defects in screen printing t-shirt products, so as to minimize product defects and losses to the company. The method used in this study is Six Sigma with define, measure, analyze, and improve phases using the FMEA method. Based on the results of data processing and analysis using the DMAI phase, 7 types of defects (cricit to quality) were found. The calculation of sigma and DPMO values is at the US industry average with an average sigma value of 4.01 with a DPMO of 6,514, which means that in one million screen-printed T-shirts produced, there are 6,514 defective products. Flaws in t-shirt products from 7 types of defects obtained 3 dominant types of defects based on the Pareto diagram, the 3 types of defects were searched for the causes of defects using a fishbone diagram and the core problem using 5W-1H. Based on the results of the RPN ranking on the FMEA method, the dominant causes of disability are caused by machine and human factors. Keywords: DMAIC, FMEA, Qualitiy Control, Six Sigma
